You can get your Form 1095-A through your HealthCare.gov account. Log in, go to your account dashboard, and look for the "Tax Forms" or "Documents" section. HealthCare.gov typically makes 1095-A forms available by late January each year.
Form 1095-A is the Health Insurance Marketplace Statement. You need it to complete Form 8962 when filing your federal taxes, which is how you reconcile the Premium Tax Credit you received during the year against what you were actually entitled to based on your final income. If you received any advance premium tax credits, you must file Form 8962 or you may face penalties or repayment obligations.
Here's where to find your form:
- Log in to HealthCare.gov
- Select your application from your account dashboard
- Click "Tax Forms" in the left menu
- Download and save your 1095-A for your tax preparer or your own records
If you don't see your form by early February, or if the information looks wrong (wrong coverage dates, wrong household members, wrong advance payment amount), contact HealthCare.gov support at 1-800-318-2596 to request a corrected form. A corrected 1095-A is called a 1095-A-C. Filing with incorrect information can create problems with the IRS, so it's worth getting it right before you file.
If you enrolled through a state-run exchange instead of the federal HealthCare.gov, your form will be in your account on that state's platform.
